The "Pathology referral form wizard" may register a second case instead of merging

We recently discovered a registration where the "Pathology referral form wizard" claimed to merge a new case with an existing case of the same laterality. Instead it created a second case item with the same laterality as the first one. To repeat:

1. Register a new case and patient with the wizard. Make sure that a laterality is selected.
2. Register a second case for the same patient. Select the option to merge with the previous case.
3. The wizard will create a new case item with the same laterality

All options for laterality was not always visible when registering a new case.
