Add a predefined option in the referral form registration when a case is registered without referral form

The background is that we have registered patients with blood samples only. In order to get INCA data for those cases we need to create a "Case" item. This is very similar to cases with a NoSpecimen item, except that we have not received any referral form.

So, the "Pathology referral form registration" wizard could be used for this, but it would be nice to have a pre-defined option (no_referral_form) that is entered in the "Reason if no specimen" field.

A checkbox was added in the last step of the wizard. If enabled, it will change the text in the "Reason if no specimen" field to "no_referral_form" and disable it for changes.
