Personal information on the SCAN-B report — at Initial Version

Description
When downloading reports with the "Gene report combiner" plug-in there is a possibility to add personal information to the report. This feature was implemented by the pilot report to add information to a specified area on the report. The SCAN-B report has no such pre-defined area so we need to erase existing information before we can add the personal information. The following headers on the SCAN-B report should be changed:
- Patient -> Personal number
- Case -> PAD (from the specimen), keep empty if no PAD has been registered
- Sample -> Replace with specimen name, make sure the yellow-specimen icon is re-added if needed
- Arrival date -> Sampling date (from the specimen)
