Create annotation type definitions for INCA 2.0

INCA has been updated to version 2.0. Most variables have been renamed. We need to create a new annotation type definitions file for creating (or updating) annotation types. The file that was used to define the current INCA annotation type can be found in /home/SCANB/BatchUpdateFiles/annotationtype.export_20160616_0945_inca_annotation_columns_-_updated.csv

The INCA_ prefix should be replaced with INCA2_.

A file with all INCA 2 annotation type definitions has been uploaded to /home/SCANB/BatchUpdateFiles/inca2_definitions_all.txt on the production server. Use this file with the Annotation type batch importer to create the new annotation types.
